Notice of Privacy Practices
The 18th Question
This page describes what this website collects. It is not yet the Notice of Privacy Practices required of a covered health care provider — that is a specific legal document with required contents, and it has to be written for the practice it belongs to. See the note at the end.
What this page collects
Two things, and only when you submit them: a phone number, and the invitation code you were given. Nothing is collected by visiting.
There is no analytics script, no advertising pixel, and no third-party tracking on this site.
What is done with them
The code is checked against the list of invitations. If it is recognised, the number is recorded and one text message is sent to it, from the number that will be used to arrange the first conversation. Nothing else is sent, and never unprompted.
Who else sees them
The message is carried by 46elks, the service that delivers it, which necessarily sees the number and the text. No one else.
Your number is not sold, shared, or used to reach you about anything other than the thing you asked for.
How long
Until you ask for it to be removed. Write to the practice and it is deleted — deleted, not marked.
What this page is not
Nothing you type here is a clinical record, and this form is not a way to reach anyone urgently. If you are in crisis, call or text 988.
